How did you find out about Keyboard Adventures?
1.  Please provide your contact information.
2.  Please answer the following questions.
Do you have a home piano or keyboard to practice on?
Describe your prior music instruction.
For the purpose of educating the public on Keyboard Adventures programs, does Mrs. Baker/ Keyboard Adventures have permission to publish photos, video, and/or musical recordings of the student participating in Keyboard Adventures activities on the studio site?
4.  Please indicate your time preferences for a private lesson.  Please select a time that is currently
clear on your schedule every week.
Private Lesson Cancellation Policy:  (change from '08 -09) With the exception of the summer session, we do not offer individual makeup times  for individual lessons cancelled by the student. Students are asked to do their best to keep their assigned weekly appointment time.  Please have the courtesy of letting the teacher know in advance that you will be absent.  If your lesson is cancelled BY THE STUDIO for any reason (weather, teacher illness, etc.), the teacher will schedule a makeup for a mutually agreeable time or issue a credit to be deducted from the next month's payment.  Masterclasses will be offered once per month, the third week of the month. The masterclass will be considered a makeup class for any lessons the student was not able to attend earlier in the month.  Students must be prepared to play one song at the masterclass.

Payment Policy: We offer pay-ahead plans and monthly installment plans.  For monthly installment plans, payment is due in full for the monthly tuition amount at the first scheduled class of the month, whether you are present or absent.  Pay-ahead plans are due on the dates provided in your payment schedule.  Payments (either pay-ahead or monthly) received after the 7th of the month are considered late.  Please add $25 to your payment if you are paying after the 7th.  We currently accept checks and cash, but we prefer checks as they provide a better record of payment.  We are hoping to accept online payments or credit cards sometime in the near future.  E-mail payment reminders will be sent, but invoices will not be mailed unless the customer's account is past-due.  Please pay in person (at class) or mail your payment to:  Keyboard Adventures, P.O. Box 606, Purcellville, VA 20134.

Practice/ Weekly Assignments:  Students will be expected to practice at home on a daily basis, at least 4 - 5 days per week, and sometimes complete a written assignment.  A "practice sheet" listing assignmnents for the week will be given to each student at each individual lesson.  If you cannot commit to a daily practice routine at least 4 - 5 days per week, please don't put piano lessons on top of your already packed schedule!  Success at any musical instrument requires several hours of disciplined, independent effort each week.

Summer Continuation/ Guaranteeing Fall Spots (Private Students):  Keyboard Adventures does not close or break for the summer.  Private students wishing to guarantee Fall placement for the next year must make a minimum investment in their instruction over the summer in order to guarantee fall placement for the upcoming school year (in 2010, this was 9 weeks out of the 11 weeks between 6/21 and 9/4).  Students who take the summer off (leave without payment) are withdrawing from the studio and will not be guaranteed fall placement.  Students may have up to two weeks of "unpaid" leave for vacation during the summer session.

Termination by the Student
To avoid owing any additional fees, please provide two weeks advanced notice in writing before the next scheduled payment is due.  You may e-mail your notice to me.
